Normally, you need something like one and a half years to produce something like that. Uh, we need... We did it in seven months. Well, in the treatment of oncologic diseases, meaning the treatment of cancers, we have, uh, very good standards. Now, why are these treatment standards? Because they are good. So, uh, we normally make a dendritic cell therapy in addition to the standard therapy. The question patients always ask me: Is it interfering, the chemotherapy, with the dendritic cell therapy? Let's say like this. The chemotherapy normally, or in general, works on dividing cells. Dendritic cells are cells that are not dividing anymore, meaning the dendritic cells cannot be harmed by chemotherapy. On the other end, more practically seen, is that the... it's always good to attack an enemy, and our enemy in this case is the tumor, from several sides. So in this pragmatic thinking, which many researchers share with me, uh, is- it is good to have the combination of a chemotherapy who goes on the division of the cancer cells, meaning, in this phase, they destroy the, uh, dividing cancer cells, and to attack them from the immunologic side of a killer cells who go m- more to s- uh, surface recognition of the tumor cells. The Laboratories Dr. Ganzerl, yeah. This means we are here in this clinic working with the patients, meaning here we take the blood and here we give the cells back. But in these seven days, which are in between, the blood is processed in our own laboratory. This laboratory underlies strict guidelines and laws from the drug law in Germany, but also the European medical agencies, the controls of, uh, different governmental structures who control that we do everything right and okay. Uh, meaning the blood, when it's taken here, it goes into our laboratory. Then all the quality assurance and quality control tests are made, meaning we look for, uh, hepatitis, for HIV, for infection criteria. We then bring the blood into the clean room, which is a, a class A clean room, meaning we produce at zero particles per cube meter. And then you must imagine, let's say we have 200 mL of blood. So first we must separate the serum. This is 100 milliliter, let's say. So from this 200 milliliter, we're now at 100 milliliter. Then we have to get rid of the red blood cells, because these ones we also do not need for the production. Then remains one mL out of these 200, which are the white blood cells. From the white blood cells, we also don't need so many, because most are neutrophil granulocytes, which we isolate then over ƒ, if you call ƒ density gradients. And then we have the mononuclear cells, meaning these are the, the monocytes plus the lymphocytes. These lymphocytes are then also isolated because we don't need them, because we just want the monocytes. Meaning out of 200 mL in the end, something remains like 0.1 mL. Uh, this one comes down into culture, and, uh, the production process of the dendritic cell, uh, the, the dendritic cells starts. This production pre- uh, process goes by adding different cytokines and growth factors. Uh, and after seven days, the cells are ready to be harvested, and then they are filled into syringes and brought back to the clinic and injected into the patients. Normally, patient get into touch with us via email or via telephone. S-- then, we first want to see the last medical reports, m- getting to know something about the patients, to see whether this patient qualifies for dendritic cell therapy. This is the first step. The second step is we try to find an appointment, because the patients must come in total three times to our outpatient clinic here in Berg, beautiful region close to Munich. Um... At the first appointment, we, uh, we make the informed consent. We explain everything again to the patient. We, uh, give also our ideas concerning the other cancer tr- therapies which are possible. Then the patient has some time to think about, and the next day, in the morning, patient comes, and we draw the blood from the patient. This complete process lasts for around two hours, because first we draw the blood, normally between 150 and 200 mL, which is no problem. And then afterwards, patient also get a vitamin infusion, and we already start giving the patient some vitamin D. Then the blood sample goes into our laboratory, and the patient has seven days off, meaning patient can either go to Munich, which has beautiful museums, a beautiful zoo by the way, or stays here in the region with a very... between Munich and the Alps. Also a very nice vacation region. Many patients always just stay for one week here to relax and calm down after all the stress. After seven days, patients come back. The dendritic cells are here already. Patients get their dendritic cells injected, and again, an infusion with vitamins and the vitamin D injection. Meaning, in total, the patient has to come three times to us as an outpatient. Uh, it's not painful despite the injections, but I think this is not a big problem. Side effects that we see are some flu-like symptoms on the day when the patients get, uh, their dendritic cells back. But what means flu-like symptoms? I always explain it that way. The feeling you have before you get the next day sick. So this not-so-well feeling. But not more, s- uh, on side effects we see. Patients can also travel, then, uh, back immediately. Some patients just come for the first two days, and then they travel back to their home countries and f- come back then after seven days. These are our fly-in-fly-out patients, which is also no problem because Munich Airport is a very big airport in Germany. It's very close t- to our clinic. So... this is the way, what the patient experience here. The other thing we also recommend to the patient, that your body has a higher need of vitamins. This you can supplement by infusions, but you also can supplement it by daily intake from vitamins, which you easily can get in your local pharmacy. On the other hand, we also must have a look that we have a higher protein need in cancer patients. For this reason, we always recommend to have a protein-rich diet for the patients. This is not only for patients undergoing dendritic cell therapy. So far most studies are made in the palliative situation. Palliative means not all tumor is taken out or away, so we try to prolong the survival of the patients or bring them into a stage that they are not palliative anymore. In the adjuvant situation, it is that we try to avoid the relapse rates, meaning the disease-free survival counts. Let's say like this. When people ask me, 'Frank, you've done so much research in your life d- during the last 30 years, even more, what was the most difficult part?' In the first decade, let's say, or the first one and a half decades, I was doing research, which is really exciting, interesting. The most hard stuff was to establish a dendritic cell laboratory which does not only produce once good dendritic cells, but which produces with an accuracy of more than 99% all the time, that it's bringing something in serial production. This was the biggest challenge in my life. This complete process took us more than 10 years to develop it. A- and I know it from also the few colleagues, I always say, not competitors, my two few colleagues here, uh, it took so long to establish everything. The reason is because if you, if you, uh, want to make a very easygoing production, you need a lot of automatization processes, and crucial steps are still bound to people's work, meaning you have a lot of very high qualified people doing the work. You cannot just write an advertiser and you can need now somebody who's, can do this. Also, these people grow with the time into their function.
